Animal Complaint Form and Log of Incidents:  use this form to report complaints on household pets and to record the frequency of nuisance incidents.

Freedom of Information Law Request: Please make your request as clear as possible in order to fulfill our obligation. When an agency receives a request, §89(3)(a) of the Freedom of Information Law states that it has five business days to grant or deny access in whole or in part, or if more time is needed, to acknowledge the receipt of the request in writing and indicate an approximate date by which the agency will respond to the application, usually not more than 20 additional business days. General Complaint Form:  may be used for most complaints to be presented to the Board for their review. If you have complaints regarding the quality of the municipal water, please fill out the form located in the Public Works Department section

Handicap Parking Permit or License Plate for persons with severe disabilities: Available at any municipal office. Certification from your doctor is required either by the doctor’s completing the certification on the pre-printed form.  Municipal Issuing Agents are required to issue permits only to qualified applicants according to where they live.  Additional information from NYS DMV regarding parking for people with disabilities can be found here:

Tax Search-Utility Billing Status Request Form: for informational purposes only, no charge; please allow at least two working days in advance

Third-Party Notification REQUEST FOR MAILING OF DUPLICATE TAX BILLS OR STATEMENTS OF UNPAID TAXES TO A THIRD PARTY, at no cost and may be used for any municipal taxing jurisdiction

Village Voucher: Signed voucher MUST be received by 9:00 a.m. the first WEDNESDAY of the month to be considered for payment approval

5-year Formal Tax Search Request Form: To be used when transferring property within the Village, Cost=$10.00; please make your request at least five working days in advance of closing as our staff is limited.  The Village Clerk must receive the fee payment of $10.00 within five days from the date of request.

Mass Gathering Permit ApplicationCosts: $100.00-$200.00 per day    Approximate Time from Application to Issue: at least ten business days before the scheduled event.  Requirements:    Details of all requirements stated on the application.  Proof of liability insurance in the amounts as indicated with the Village of Fonda named as additional insured.

Costs: $30 Application Fee Plus – $100-$250 Permit Fee

Public Works Department

Forms that deal with water, sewer, garbage and debris removal, sidewalks, street openings, and excavations can be found here. Most of these forms require the payment of a fee; refer to instructions on the forms.  Approximate time from application to Issue: Generally, allow 5- 10 business days prior to onset on any action

Water-Sewer: New Water Connections-Cost is variable, with minimum tapping fee plus expenses; approximate time from application to issue: at least 10 business days prior to scheduled work.

Water Turn On Service Request: Cost=$30.00, to be paid before the water is restored;

Water Turn Off Service Request: Cost=$30.00 to be paid before the work is performed.

Seasonal Water Service: Please allow 5-7 working days in advance of date to suspend/restore water services; Cost=$10.00

Disputed Water -Sewer Billing Charges: Upon receipt of the Water Bill Complaint Form, the statements will be reviewed by the Public Works Manager, the supervisor in charge of water meters, the Finance Manager, the Water Clerk and any additional Village staff necessary to reach a conclusion. During the time period of investigation, the account will be placed on management hold to avoid shut-off or any legal ramifications; must be received for review within 30 days of bill date.

Water Meter Test Request: Consent to have your water meter tested for accuracy and calibrated to within accepted guidelines set forth by the AWWA (American Water Works Association). Cost=$ 100.00 minimum, depending on calibration site.

Water Quality Complaint Form: Free to submit for review;

New Sewer Connection Request: Cost: $500-$1,500.00

Sewer Service Disconnection Request: Permanent: $500.00; Having the sewer permanently disconnected from the sewer main will render the structure to be deemed as “uninhabitable” and proper notification will be made to the NYS Department of Health.   Temporary suspension: $40.00

Dump Truck Usage Consent and Agreement Form: Cost=$25.00 minimum, which includes first 400# of disposal fees and mileage from inside Village to the Randall Transfer Station   The usage of the Village owned dump truck is a service we wish to afford the residents of the Village of Fonda under certain guidelines.

Sidewalk, Driveway, Street Opening, Digging and Excavation Permit: Cost=$75-$250.00; Approximate Time from Application to Issue: at least 10 business days prior to scheduled event.  Requirements:    Details of all requirements stated on the application.  Proof of liability insurance in the amounts as indicated with the Village of Fonda named as additional insured.
