This clinic will provide you with the first dose of a two dose series of the Pfizer COVID-19 Vaccine. The second dose will be administered on June 3, 2021. If you cannot attend the second dose date do not register for this clinic.
Also, if you have a history of experiencing an allergic reaction to a vaccine or any other injectable medication do not register for this clinic. If you are pregnant, do not register for this clinic.
On the day of the event please do not arrive any earlier than 5 minutes before your appointment time, wear a short sleeved shirt, have photo identification with you and your confirmation ID number available. Anyone under the age of 18 must be accompanied by a parent or guardian.
